What does "pay only for outputs that pass" mean?
Pass/fail criteria are agreed upfront, based on product fidelity. Any output that meets them, you pay for. Any output that misses on fidelity is regenerated, so you don't pay until it passes. Visuals that change the product are misses, not passes, so you don't pay for them, and aesthetic preferences are handled separately since they aren't fidelity misses.
